Automated Care Plans
We understand that providing consistent, edvidence-based medical treatment is a key componant of quality healthcare. That’s why the QPathways care management platform is built around Automated Care Plans that guide each patient through their care pathway, step by step.
When a patient is activated within QPathways, their intake and assessment pre-populates that tests, surgieries, rehab sessions, check-ins, or follow-up appointments that are right for them. If, along the way, a new symptom comes up, or a new complication is identified, QPathways automatically updates their Care Plan with new steps based on their physicians protocols.
Throughout the process, each patient’s Care Manager uses the Care Plan to track patient progress, conduct follow-up calls, and enter assessment data.
