Does care navigation replace my referral or treatment plan?
No. Care navigation supports the care you provide. It doesn't replace your clinical judgment, and member benefits and referral or authorization requirements stay the same.
Is there a cost to my patient?
Care navigation is free to eligible members. Visits with providers are subject to the member's copayment and deductible.
Which patients are eligible?
Members with recurring, persistent or ongoing urological conditions. Today the service is available to a select group of Blue Cross Blue Shield of Michigan members; starting January 1, 2027, it expands to all Blue Cross and BCN commercial fully insured members with PPO and HMO plans. Members can confirm eligibility by calling the Customer Service number on the back of their member ID card.
What conditions does the team support?
Urinary tract infections (UTIs) and kidney stones.
Does TRIARQ Health provide medical care?
