Care Pathways: Pre-Op to Full Recovery
After your initial pre-op assessment with the patient, you select the evidence-based care pathway best suited for them. All of the necessary orders you’ll need have been standardized and drafted ahead of time.
The care pathways include a simple and standard post-acute care plan, such as outpatient PT or home health orders.
Our team of local nurses and care coordinators track patient progress throughout the 90 days of care. Our QPathways App lets our nurses, you, or your staff communicate directly with patients, at your discretion, without making them come into the office.
We work with you to manage these pathways so that you can focus more on treating patients and less on wrangling post-acute care providers and paperwork.

Formalizing & Digitizing Your Orders
Everything starts with you, the physician. You determine the patient’s care pathway and you sign the orders. We’re simply creating structure and formalizing the communication between participating providers to ensure clarity.
Once you sign the order determining what kind of post-acute care the patient will receive, copies are distributed to all participating providers from the hospital to the PT organization.
Participating Providers then follow the care pathway ordered and and submit documentation to maintain a clear paper trail that TRIARQ audits for compliance.
